Health and Safety Representative (HSR) Identification Card
If you are an elected Health and Safety Representative (HSR) who has completed the training, you can apply for an identification card.
To apply you will need to submit the following documents to wst.licensing@justice.tas.gov.au
- Application form
- Evidence of your election as a HSR
- A copy of your HSR training Certificate of Attainment
- A passport sized photograph in JPEG format
