Why and how to investigate an incident

Investigating incidents that happen in your workplace is an important way for you to make your workplace safer, by finding and fixing the causes of problems to stop them from happening again. Incidents you should investigate include any breaches of your policies and procedures, and safety and hazard issues (including psychosocial hazards).

The main purpose of an investigation is to prevent future incidents. This means looking at what should have happened and comparing it to what did happen to identify areas that need changing.

A clear and thorough investigation helps improve safety and create a better work environment for everyone.
