The Mines Work Health and Safety (Supplementary Requirements) Act 2012 requires a mine holder to appoint a mine operator before any mining operations are carried out at the mine.
Use this form to advise the Chief Inspector of Mines in writing of that person’s details and the date the appointment takes effect within 14 days after making that appointment.
