Licence to Perform High Risk Work Renewal

Please complete this form if you wish to apply for renewal of your licence to perform high risk work.

Lodge this completed and signed application in person at a Service Tasmania outlet. You will need to bring:

  • Evidence of identity documents
  • Fee $64.08

Note: if your application is not completed correctly or do not have the required documentation it will not be accepted.

Have you ever been convicted or found guilty of any offence under the Work Health and Safety Act or Regulations 2012 or under any equivalent work health and safety law of another State or Territory?
Have you entered into any enforceable undertakings under the Work Health and Safety Act or Regulation 2012 or under any equivalent work health and safety law of another State or Territory?
Have you had any licence issued under an equivalent work health and safety law been suspended, disqualified, cancelled or had condition(s) imposed?
Have you had any licence issued under an equivalent work health and safety law refused?

Application information

You should complete this application form if you are applying to RENEW your licence to perform high risk work.
If you want to renew your licence while temporarily overseas or interstate please call our Helpline 1300 366 322 for advice.
Interstate licence holders, who now reside permanently in Tasmania will need to complete the new licence application form
(GF083).

Lodging your application form

To lodge your renewal application you will need to take it in person to a Service Tasmania outlet. The Service Tasmania
officer will:

  • make sure that your application form is completed correctly and you have all supporting documentation
  • verify your identity
  • take your photograph
  • accept your application fee and provide you with a receipt

Service Tasmania will forward your application to Worksafe Tasmania. You will receive your new licence in the mail at your
nominated postal address within 4 weeks.

If you do not receive your new licence within 60 days please contact Worksafe Tasmania on 1300 366 322 or (03) 6166 4600
if outside Tasmania. Failure to do so may incur a replacement licence fee.

Evidence of identity requirements

Please provide at least:

  • one document from Category A plus two documents from Category B or
  • two documents from Category A plus one document from Category B

Acceptable forms of identification include the following:

Documents category A

(All documents must be original and current)

  • Australian Birth Certificate (not an extract)
  • Citizenship papers
  • Australian Passport
  • Overseas Passport/Visa
  • Australian Driver Licence
  • Australian Firearm Licence
  • Department of Immigration and Multicultural Affairs issued travel documents, including a Visa
  • Department of Immigration and Multicultural Affairs issued Certificate of Evidence of Residential Status

Documents category B

(All documents must be original and current)

  • Medicare Card
  • Plastic Bank or Credit Card, with signature
  • Tertiary Student Identity card
  • Department of Veterans’ Affairs/Centrelink Pensioner Concession Card
  • Births Deaths and Marriages Change of Name/Marriage Certificate (to prove name change)
  • Security Guard/Crowd Controller Licence
  • Government issued Personal Photo ID Card

Personal information we collect from you for licensing purposes will be used by WorkSafe Tasmania for that purpose and may also be used for related purposes permitted by the Work Health and Safety Act 2012 and associated laws. If you do not provide this information, your application may not be processed or records not maintained properly. Your personal information may be disclosed to third parties such as law enforcement agencies, courts and other government bodies authorised to collect it. This information will be managed in accordance with the Personal Information Protection Act 2004 and you may access it on request to the Department of Justice (you may be charged a fee).
